MEMber
- Licensed healthcare professional accredited to administer injectables
- Proof of current malpractice insurance
- In good standing with state licensing board
- Successfully completed at least 16 hours of accredited CE approved clinical injectable training
fellow
- Fulfilled the requirements of Membership
- Successfully completed at least 32 hours of accredited ADA or CME clinical injectable training
- Submitted a combination of 6 Botulinum Toxins & Dermal Filler cases accompanied by: Written review, before/after pictures, and documented treatment planning
- Passed the review & approval by the Board of Directors after case submission
master
- Fulfilled the requirements of Fellowship
- Successfully completed at least 48 hours of accreditedADA or CME clinical injectable training
- Attends annual update meeting on nonsurgical aesthetics -IAFA symposium or similar
- Actively administering facial aesthetics for at least 2 years
- Submitted 10 documented advanced before/after Botulinum Toxins& Dermal Filler cases
- 1 written article or case study on facial aesthetics in either print or digital form – Fellows can submit articles to the IAFA for publication.
- Successfully passed the Mastership written examination
diplomate
- Fulfilled the requirements of Mastership
- Successfully completed 60 hours of accredited facial aesthetics continuing education courses and attends at least 16 CE hours per year
- Successfully completed a cadaver anatomy training for facial injectables
- CV demonstrating a minimum of 5 years clinical practice experience
- Submitted 15 documented advanced before/after Botox & Filler patient cases (see below for requested breakdown)
- 5 Advanced Botulinum Toxin or Combo Neurotoxins with fillers
- 5 Facial Sculpting with dermal fillers or Neurotoxins with fillers combo
- 3 dermal filler cases that were combined with one of these additional treatment modalities: PDO threads, PRF, Lasers, Sculptra and/or RFMN
- 2 written articles or case studies on advanced facial aesthetics in either print or digital form- can submit articles to the IAFA for publication.
- Score of 75% or higher on Diplomate Proficiency Exam
